Zoonotic dermal leishmaniasis
Zoonotic dermal leishmaniasis Synonyms: the second type of disease Bohr, acutely necrotizing the form, a rural type. It is characterised by shorter incubation interval which proceeds from several days till 3 weeks. On a place of introduction of the originator there is a conoid process in diameter of 2-4 mm which quickly grows and in some days reaches 1-1,5 sm in diameter. In the hillock centre there is a necrosis. After tearing away of necrotizing tissues the ulcer in diameter of 2-4 mm with steep edges is discovered. Round it a wide infiltrate and an edema. antroponose a leishmaniasis it differs from an initial hillock in the big size, a bright redness, edema presence in a hillock and round it, a tachyauxesis. Sometimes reminds a furuncle in an infiltration stage, differing from it larger softness and small morbidity. After appearance the ulcer quickly extends at the expense of a necrotization of an infiltrate along the edges of an ulcer. Individual ulcers sometimes happen rather extensive, diameter to 5 sm and more. At plural ulcers, and at this type of a leishmaniasis their number can reach several tens and hundreds, the dimensions of each ulcer are insignificant. Ulcers have the rough saped edges, the bottom is covered by necrotic masses and the abundant is serous-purulent abjointed. By 3 month the ulcer bottom is cleared, granulations expand, reminding a papilloma. After tearing away of superfluous granulations there is a characteristic rough surface on which there are cuticularization islets (more often from the ulcer centre). Process comes to an end in 5 months lymphangites, lymphadenites, consecutive leishamaniomas are quite often observed. Abortal flow of a leishmaniasis when an open ulcer it is not formed is sometimes observed and in 1-2 months there comes a granuloma cicatrisation under a crust. At both types of a dermal leishmaniasis the chronic tuberculoid form reminding on flow and implications a lupus can educe. Process can proceed till 20 years. Basic element at this form is the small hillock (2-3 mm in diameter) zheltovato-brown colour. Hillocks are often localised on the person in the form of the solitary elements or merge in a continuous rough surface. Separate hillocks canker, and along the edges of an infiltrate new hillocks are formed. Thus, the infiltrate grows on periphery. After an ulceration of hillocks there are cicatrixes.
Complications. Consecutive infection apposition enhances morbidity and inflammatory changes. At lymphadenites on the inferior extremities edemas of anticnemions and autopodiums at the expense of a lymphostasis can be observed.
